What is Community Rehabilitation
Service?
• Community rehabilitation service falls under the umbrella of intermediate
care services.
• The aim of the service is to support clients to recovery faster from illness or
trauma, who are discharged from hospital or to prevent clients going into
hospital and to ensure they return to independent living.
• It can be delivered as:
• Bed based rehabilitation - this is normally within care homes (temporary
basis) e.g. LICC (Lisburn), Northfield (NDA), Ardview (Downe).
• Home based rehabilitation - this is within client's own home, including
permanent residents in care homes, or may be on a temporary basis
living with a relative.
• It is a multidisciplinary (MD) service consisting of Occupational Therapists
and Physiotherapists supported by Rehabilitation Assistants.
• The service is offered for up to 6 weeks - if you think the client needs to be
kept on longer this MUST be discussed with team lead.
Criteria
• 18 years and over
• Currently residing in SET
• Identified rehab goals by physiotherapist/ occupational therapist
• Medically stable to participate in rehabilitation and not likely to deteriorate with intervention
• Motivated and able to engage with rehabilitation
• Most clients will require both physiotherapy and occupational therapy input
• Exceptions - OT only Rehab -under 65's , Fracture rehabilitation, requires assistance of 2 staff,
bed based rehab service
• Physio only- Fracture rehabilitation, bed based rehab service
Exclusion criteria - rehabilitation that can be provided by specialist teams ( eg palliative, stroke,
brain injury)
