You and the police
The police take the lead role in investigating suspicious
deaths to determine if there is sufficient evidence to
support a charge of corporate manslaughter and/or
gross negligence manslaughter. This workshop is aimed
at managers, including:
• the legal framework
• your organisation as a suspect
• individuals as suspects
• the organisation and staff as witnesses
• disclosure of information to the police
• supporting staff as witnesses during criminal
proceedings.
When the CQC inspector calls
The role of the CQC has been extended to include inspection of premises, formerly the remit of the HSE, and a
series of new fundamental standards have been published. Having a plan to manage a CQC audit is essential. This
workshop is aimed at managers and includes:
• the legal framework and CQC role and powers
• new fundamental standards and outcomes data
• preparing for and managing the initial visit of the CQC
• conducting your own internal investigations
• ongoing liaison with CQC throughout the investigation
• responding to the imposition of conditions/restrictions
• preparing for an appeal
• avoiding criminal prosecution.
Medical records and court processes
A workshop aimed at helping practitioners realise the
importance of accurate records, including:
• real life case studies - records under scrutiny
• the principles of court processes - clinical
negligence claims and inquests.
Regulator investigations
It is inevitable that you will come under the spotlight
from one of the regulators from time to time either
following a suspicious death, never event, even a near
miss or following a statutory inspection. This workshop
ensures that your people understand the following:
• who the regulators are
• the legal framework under which they operate
• how they may be involved with the trust
• their powers
• what an investigation involves
• the key steps in managing an investigation.
Root cause analysis
A root cause analysis is essential to preventing the
same incident being repeated. It can also have wider
applications, which this training will explore.
Based on a real-life scenario, this interactive
workshop will use root cause analysis tools to analyse
an incident. The session will explore reporting
styles and information gathering techniques for any
investigation.
