MULTIDISCIPLINARY TEAM
Below is a short description of the care the multidisciplinary teams provide:
Consultants:
The allocated medical leads that are responsible in the overall care of the patients while in
hospital.
Other Medical Staff:
The Associate Specialist and Junior Doctors assist the Consultants in the day-to-day care and
management of the patients. Other medical teams that patients may encounter during their stay
are the Specialist Trainees and Locum Doctors.
Ward Sister / Charge Nurse:
These are the most experienced and senior nurses on the ward, they manage and oversee
resources and ensure the care of patients is maintained through National guidelines, policies
and procedures.
Staff Nurses:
Are registered nurses who plan and evaluate your care. There will be a Registered Nurse
looking after a group of patients day and night.
Nursing Assistants:
Support the Ward Sister/ Charge nurse and registered nurses in delivering and implementing
your care.
Pharmacist:
Clinical Pharmacists work directly with the Medical team and the wider multidisciplinary team
to ensure that the medications prescribed for patients contribute to the best possible health
outcomes.
Occupational Therapist:
Occupational therapy is a client-centred health profession concerned with promoting health and
Wellbeing through occupation. Occupations are the tasks people need and like to do and include
Self-care, work and leisure participation. Following illness or injury the ability to perform everyday
Occupations can be affected. The primary goal of occupational therapy is to help people regain
As much independence as possible in the occupations which matter most to them.
Physiotherapist:
Physiotherapists help people affected by injury, illness or disability through movement and
exercise, manual therapy, education and advice.
Social Workers:
Individuals can be referred to adult services who will assess and ensure care requirements are
met on discharge. They provide support and advice to individuals and families if required.
Speech and Language Therapist:
Will assess you if the team are concerned about your ability to swallow fluids and food safely.
They may recommend that your diet and fluids are modified.
Dietician:
Will assess you if you have lost weight or have a reduced appetite. They may recommend
nutritional supplements.
