Dementia Navigator Service
Dementia Navigators provide a highly responsive, individualised
information, signposting and awareness raising service throughout the
dementia journey, developing meaningful partnerships and supports, so
that people living with dementia and their families/carers can improve
their health and wellbeing and take control of their health.
The team includes nurses and social workers and is therefore highly equipped to provide a
holistic response, promoting effective inter-disciplinary and inter-agency relationships to ensure
continuity of care.

The work of the Navigators has been showcased at the Dementia Together NI Celebration in
November 2017 and was presented to the Trust Reform Board in February 2018.

Dementia Together NI performed a regional evaluation of the Dementia Navigator Service and
found South Eastern HSC Trust to be one of the leading Trusts.
