Unscheduled Care - Transition Ward
In August 2018 the Ulster Hospital opened a brand
new ward call the Transition Ward.
This unit is a nurse led facility that focuses on
complex discharges in the hospital and allowing
patients to be accommodated in one area in order
to focus on social rehabilitation while they transition
from an acute stay in hospital to another level of
care.
The unit initially opened with 10 beds and quickly
this increased to 13 beds.
In January 2019 the unit increased to 26 beds. The unit has been successful in recruiting registered
nurses, senior nursing assistants and nursing assistants.
We focus on the individual and giving them and their family ownerships of their care while in the unit.
A significant amount of our patients live with dementia and delirium so for this reason we are working
towards a dementia friendly environment.
We have regular social activities on the ward from pet therapy, games, music, choirs, reminiscence,
nail care, Indian head massage and high tea parties. The aim is to get patients out of bed and into
day clothes and focus on what they like to do as an individual, thus reducing all hospital associated
mortalities.
We are almost finished our Day Room which will encourage social interaction and allow patients to
eat in a communal area.
We have scrapped visiting times on the ward in order to allow families ownership of care and allow
them to participate in care planning. We have signed up to the #John'sCampaign, meaning there
are no restrictions on care or visiting for those living with dementia.
We have had the unit painted in dementia friendly colours to make the journey a little easier for those
we care for.
Patient and family feedback has been outstanding with the ward regularly achieving 100% in the
friends and family feedback KPI's and high 95%+ in the friends and family recommendations.
