The revised Person-Centre Nursing Assessment document for use in
the adult inpatient setting has been developed and regionally agreed
with the professional guidance of NIPEC (Northern Ireland Practice and Education Council for
Nursing and Midwifery).
Recording Care
'The purpose of recording nursing care is to evidence decision making and clinical
judgements while supporting delivery and continuity of care, subsequently improving
communication between healthcare professionals and the identification of risks to the
person accessing the service.
Nurses are required from a regulatory and employer perspective to provide evidence of
their contribution, professional judgement and interventions in care delivered.
Recording their contribution is an integral part of nursing and midwifery care'.
