Aim
To increase awareness of the
supports available to young carers
within the Physical Health &
Sensory Disability (PHSD) Social
Work Team from 30% to 100% by
September 2019.
• Increased awareness of supports
available to young carers
• Improved the quality of service
delivery and support to young carers
• Developed a consistent approach when
working with young carers
• Co-Produced/Co-Designed a resource
booklet in partnership with young
carers. What has it achieved? What are we doing and why?
Our project improved awareness of
resources available for young carers within
our team. Through engagement with the
team and young carers, we expanded our
focus to co-design/co-produce a resource
booklet. The resource will promote a person
centred approach and working in
partnership with young carers and families.
Data collected from pre and post questionnaires.
