Adverse Incidents
An Adverse Incident is defined as:
'Any event or circumstances that could have or did lead to harm, loss or damage to people,
property, environment or reputation' arising during the course of the business of a HSC
organisation/Special Agency or commissioned service'.
The Trust is committed to providing the best possible services for its patients, clients, visitors
and staff. It recognises that while adverse incidents will occur, it is important to identify causes to
ensure that lessons are learnt to prevent recurrence. To this end, it is essential that a responsive
and effective incident recording, reporting and management system is in place.
We encourage this by providing an open, no blame, learning culture where, when learning from
such adverse incidents is identified, the necessary changes are put in place to improve practice.
Learning and sharing from adverse incidents can only take place when they are reported and
investigated in a positive, open and structured manner, enabling the Trust to reduce risk and
proactively improve services.
Facts & Figures
During 2019/2020 there were 15,388 incidents reported, 11,587 of which were patient related
incidents.
The top 5 most reported incident types are shown in graph 1 below in comparison with 2018/2019
in graph 2.
