Enhanced Care at Home

The Enhanced Care at Home Service (ECAH) has been
developed through an Integrated Care Partnership (ICP) to
provide person-centred care for acutely unwell individuals.
It allows people to stay at home as an alternative to
hospitalisation. Additionally, it can facilitate an early hospital
discharge.
A person's need for therapeutics, assessments and
monitoring are provided in the comfort of their own home
with agreed goals and interventions.
• It's a short, responsive Multi-Disciplinary Service consisting of Consultant Geriatricians
who support our GP's, Senior Nurses, Nursing Staff, Senior Nursing assistants, Band 7
Physiotherapists, a Pharmacist and Occupational Therapists who are available from
Community Rehabilitation Team
• ECAH has been operating in all three Trust localities since June 2018.
2,958 bed days have been saved
89% of completed ECAH interventions avoided hospital admission
