Home Oxygen Assessment and Review (HOSAR) is a Regional Oxygen Service that ensures all
patients who have been prescribed oxygen will have:
• Risk assessment
• Education
• Monitoring
• Referral to Community Respiratory Team to ensure ongoing support while they require
oxygen.
This service is for respiratory and non-respiratory patients.
South Eastern HSC Trust currently has a robust HOSAR Service in Community, Lagan Valley and
Downe Hospitals.
No HOSAR Service was available within the Ulster Hospital for patients being discharged on home
oxygen in North Down and Ards locality.
Respiratory Nurse Specialist, Joanne Gamble led an SQE Project using quality improvement
methodology to improve patient safety following discharge.
This initiative was successful in ensuring that HOSAR was implemented for the project period.
Achievements
100% patients discharged from Ward 13 in the Ulster Hospital with oxygen therapy had been
consented to treatment, had a risk assessment completed and a referral to the Community
Respiratory Team.
It is planned that the improved HOSAR Service will be extended to the entire Ulster Hospital site. 12
